In connection with my application for employment or ongoing employment with the Company, I understand that a "consumer report" as this term is defined in the Fair Credit Reporting Act ("FCRA"), as amended15 U.S.C § 1681 et seq., may be obtained by the company from a consumer reporting agency. I understand that "consumer reports" include, but are not limited to, credit reports, criminal background checks, andmotor vehicle reports.

I further understand that the Company will provide a copy of the consummer report to me if the information contained in such report is, in any way, to be used in making decisions regarding my fitness for employment or ongoing employment with the Company. I understand that such report will be made available to me before any such decisions are made, along with "A Summary of Your Rights Under the Fair Credit Reporting Act."

By typing my name below and clicking "I have read the terms of agreement", I here acknowledge that I have read the above disclosure and voluntarily authorize the Company, including its agents and representatives, to obtain a consumer report on me for use in connection with my application for employment or ongoing employment with the Company. If hired or currently employed, I understand that this authorization will remain on file and shall serve as an ongoing authorization for a consumer report to be lawfully obtained at any time in connection with my employment.

I have read the Terms of Agreement:

"I certify that the facts contained in this application are true and complete to the best of my knowledge and understand that if employed, falsified, statements on this application shall be ground for dismissal. I authorize investigation of all statements contained herein and the references listed above to give you any and all information concerning my previous employment and my pertinent information, they may have personal or otherwise, and release all parties from all liability for any damage that may result from furnishing same to you.

"I understand and agree that, if hired, my employment is for no definte period and my, regardless of the date of payment of my wages and salary, be terminated at any time without prior notice."
